Hello Woodfield Family:

You are invited to participate in our survey "Woodfield Academy Dress Code". In this survey, approximately 65 people will be asked about their thoughts on the dress code. It will take approximately five minutes to complete the questionnaire.

This survey gives you three choices of dress codes you would like to see at Woodfield Academy. Please carefully read all three choices and choose one that you think fits our school the best. No decisions have been made regarding dress code, we are just doing some research to see if a change needs to be made.

Your participation in this study is completely voluntary. There are no foreseeable risks associated with this project. However, if you feel uncomfortable answering any questions, you can withdraw from the survey at any point. It is very important for us to learn your opinions.

Your survey responses will be strictly confidential and data from this research will be reported only in the aggregate. Your information will be coded and will remain confidential. Choice 1

1. Shirts - all shirts must have a collar and be plain white, burgundy, or gray and must be tucked in throughout the day (for girls, fashion fit shirts are not permissible). Undershirts must be plain white, burgundy, gray, or black.

2. Pants - dress type khaki, navy blue, or black pants/shorts, belt worn if designed for a belt. (capris and cargoes are not allowed). Shorts/skirts must be knee length.

3. Skirts/skorts - girls in grades 2-5 may wear khaki, navy blue, or black skort. Skirts and skorts must be approximately knee length. Girls in grade 6-12 may wear khaki, navy blue or black skirts. Skirts and skorts may only be bought at Buckhead Uniforms.

4. Belt - a solid color navy blue, black, or brown belt is worn. Braided leather or canvas belts are allowed. Belts must fit and be free of ornamentation.

5. Shoes - Shoes may be solid brown or black leather shoes, tennis shoes that are black, blue, gray, white, or a combination of these colors may be worn. NO OTHER COLORS ARE PERMITTED, REGARDLESS OF AMOUNT.

6. Sweaters/sweatshirts - students may wear a plain gray, white, burgundy cardigan sweater, fleece, or sweatshirt. When wearing sweatshirts, shirt collars should be visible outside of sweatshirt (No hooded sweatshirts allowed). During cold weather, an additional coat or jacket may be worn on top of the sweatshirt while students are outside.

7. Watches and accessories - girls and boys may wear a watch. No baseball caps or sunglasses may be worn during school hours. Girls may wear one pair of earrings. Jewelry should be kept to a minimum.

8. Personal appearance - hair may not hang below eyebrows, for boys, may not touch the shirt collar. Hairstyles may not be distracting to other students or faculty. Boys must be clean shaven every day. Body piercings and tattoos are not permitted. The faculty and staff reserve the right to determine which hairstyles are distracting.

9. Dress down Friday - every Friday students will be able to wear jeans and a plain colored t-shirt or collared shirt to school along with the shoes of their choosing. Jeans must fit at the waist and may not have holes or frayed edges. T-shirts must have sleeves and cover the midriff. No writing is allowed on t-shirts.

2. Pants - jeans or khakis may be worn. All pants must be free of holes and frayed edges. Pants must also fit at the waist and cannot be overly baggy or tight.

3. Shorts - jean and khaki shorts may be worn. They must be knee length, free of holes and frayed edges. Shorts must also fit at the waist and cannot be overly baggy or tight. GYM SHORTS ARE NOT ALLOWED.
