Dear Western Cape Entrepreneurship Awards participant

BRIEF BUSINESS PROFILE INFORMATION -- FORM WILL TAKE 15 TO 20 MINUTES TO COMPLETE IF CONCISE ANSWERS ARE PROVIDED

Thank you for submitting your business as a potential Awards winner. The purpose of this electronic form is to give the Awards judges a general profile of your business.

All information provided on the form, with the exception of the "short marketing profile" will be kept confidential, and judges will be required to sign a Non-Disclosure Agreement. The selected judges will be experienced business experts of a high ethical calibre.

The total possible points for each section is indicated. The final score will be out of 100. Judges will base their score on the information provided. If you are one of the selected 30 finalists, you will have the opportunity to present your business profile to a panel of independent judges – this panel will then select the final Award recipients.

If you have any queries regarding this form, kindly email the Awards Adjudication Project Manager, Desi Nair, at [email protected].

Thank you very much for your time and support. Please start with the survey now by clicking on the Continue button below.
