1. Hello:
I invite you to take part in a research study: Tornado Preparedness for People with Mental Disabilities in Arkansas. This study seeks to find out the preparedness levels across Arkansas in the event of a tornado and how people with mental disabilities are cared for during and after the tornado. You are being offered the opportunity to take part in this research study because you are enrolled in the class Research Design and Methods at Arkansas Tech University. If you decide to take part in this study, your information will be used to evaluate the average preparedness levels in the state of Arkansas and to inform me what needs to be done in order to raise the number of people that are prepared for tornadoes. Your participation in this study is entirely voluntary. If you decide to participate in this study, you must select

below to show that you have chosen to take part. If, at any time during the study, you wish to no longer participate, you may stop right then. If you decide to participate in this study, your involvement will last as long as it takes to complete the study questionnaire, which includes 10 questions. There are no risks associated with this study. You may benefit from the results of this research in that a caregiver of a person with a mental disability may be better prepared for a tornado. You may be that caregiver, the person with the mental disability, or a relative/friend of the person with the mental disability. In the event of any publication or presentation resulting from the research, no personally identifiable information will be shared. You will not receive any compensation for being in this research study. This is an independent study and is not funded by any company or organization. I urge you to contact me with any questions, comments, or concerns you may have in regards to this study and your participation in said study. You can reach me, Tiffany Cottrell, anytime at
