Please answer the questions below based on your activities within the office. Some questions may require an answer based upon your impression of the general practices of the workplace. Also, the words office and workspace can be used interchangeably depending on if you are in a cubicle or if you have an office. Feel free to skip any questions you feel you are unable to answer.
