Substance abuse and medical services

Hello: You are invited to participate in our survey related to the use of medical services during active substance abuse. It will take approximately 5-10 minutes to complete the questionnaire. Your participation in this study is completely voluntary. There are no foreseeable risks associated with this project. However, if you feel uncomfortable answering any questions, you can withdraw from the survey at any point or leave any question unanswered. It is very important for us to learn about your expereinces in an effort to help others who may be chemically dependent. Your survey responses will be strictly confidential and data from this research will be used to provide education to professionals who work with individuals who may be experiencing issues related to substance abuse. Your information will remain anonymous.
