This brief survey is designed to see how you perceive intelligence and the work you do in school.
Your answers will be extremely useful in helping us examine how students view their mathematics school work and how that relates to interest in science. Please take your time to read all instructions carefully and to answer the questions to the best of your ability.

We will only keep the e-mail address on the survey and we will not know your name.
We will not share individual answers with your teachers, peers, or counselors.
Your e-mail address will be used to destroy your survey if you decide you want to withdraw from our study. The information will remain confidential, and once analyses of data are finished, your survey will be destroyed. Any analysis of data will result only in group information and will be reported as such.
