Exit Survey

Thank you for your interest in taking our survey to help us evaluate the feasibility of a multi-modal trip planning tool for San Joaquin, Stanislaus, and Merced Counties. All of your answers to the survey will be kept strictly anonymous and confidential.

As a token of our appreciation for your time, participation and opinions, by completing the survey, you will have a chance to win one of: Six Bluetooth Headsets, One $100 Gas Card, Two $50 Gas Cards, and Six $25 Gas Cards. That’s 15 total prizes! One prize (headset or gas card) per winner.

The winner will be notified in late-March and you need to provide only your email at the end of the survey to be entered into the drawing. Again, all of your answers to the survey will be kept strictly anonymous and confidential.

This survey will take about 12 minutes to complete, and it includes four sections: issues of importance; transportation habits; computer/device and trip planning tool use; and demographics.

Once you have answered all the questions on a page, click the “Next” button in the lower-left corner of the screen to continue. If you have any technical difficulties with the survey, please email: Technical Assistance or call 650-288-3031.
