INFORMED CONSENT FOR EDUCATION You have been selected to complete the online survey Teacher Motivations and Perceptions of Implementing Alternative Seating Hello! You have been asked to participate in an online research survey that is part of the requirement for a Specialist degree in Educational Leadership for Kelli Hartman at Southwest Baptist University. The questions in this survey will ask about your knowledge of alternative seating, your motivations for implementing or not implementing and your perception of its effects on off task behavior. You are being asked because teachers today in both large and small districts are implementing alternative seating at different rates and its effectiveness is not yet been determined. This survey is being sent to two different sized conferences in Southwest Missouri. Consent and Eligibility You must be 18 years or older to participate in this survey. Your completion of this survey is voluntary. The survey should take less than 20 minutes to complete. Your answers will be kept anonymous: they will not be associated with your e-mail address or be communicated back to your school administration. You may choose to not answer any questions or stop at any time. At the end of this e-mail is a link to the survey.
