In preparation for development of the 2012 Sales and Underwriting incentive plan design, we are looking for your feedback and suggestions to ensure our incentive plan is aligned with our strategic objectives, organizational model, and the evolving health care environment. Please complete this survey by end of day Thursday, May 5th. The survey results will be presented to the incentive design team during our planning meeting on May 9th.
The survey should take no more than 10 minutes to complete and it's broken into two parts - feedback on the current plan and considerations for the 2012 plan design.
