Hi,

As an integral part of my PD, I would like to ask you as a colleague to provide me with feedback on your experience and interaction with me over the past 12 months.

As your honest feedback is important to me to further my development, I have opted for an anonymous approach to providing feedback.

Naturally should you wish to convey or even discuss further your feedback with me, there is the option to provide your name within the free text box at the end of the questionnaire. That way we can ensure the possibility to discuss the key feedback points you would like me to work on.

The questionnaire should not take you more than 10 to 15 minutes to complete.

When going through the activities, please relate these to my role as a P&S Manager and our interaction during one or more of the projects we have worked on together.
