Hello: You are invited to participate in my survey about sleep. Thank you for your time and consideration in helping me gather information. The data will initially be used as part of an NFA project. My hope is to use the data to identify areas of improvement as it relates to us, Richfield firefighters, and sleep. In this survey, approximately 25 firefighters will be asked to complete a survey that asks questions about sleep, both on and off duty. It will take approximately 2 minutes to complete the questionnaire. Your participation in this survey is completely voluntary.. If you feel uncomfortable answering any questions, you can withdraw from the survey at any point. Your survey responses will be strictly confidential and data from this research will be reported only in the aggregate. Your information will be coded and will remain confidential.
