My name is Susan Huerta I am currently a first year student at George Brown Collage in the Clinical Methods in Prosthetics and Orthotics. I have had interest in prosthetic methods and would like to begin my own research project on the layups.
You are invited to participate in my online survey that will address your personal use of different combination of lamination layups. In this survey, I will be contacting facilities across Canada and abroad. Approximately 100 facilities will be contacted. Questions will mainly address personal uses of layups combinations and the reasoning behind these choices.
It will take approximately 10-15 minutes to complete the questionnaire.
Your participation in this study is completely voluntary. I would just like to ask if information about layup composition is purposely left out for any reason that it is mentioned. If the composition of the layup is confidential to your facility please check that off in the appropriate box and it will not be disclosed.
If you feel uncomfortable answering any questions, you can withdraw from the survey at any point. It is very important for us to learn your opinions.
Your survey responses will be used in further research and will be presented in the Grand Rounds in the following year.
