Purpose of this survey

As one of the key stakeholders involved in supporting the implementation of the FFSR KIT Capability Roadmap, the feedback you provide in this survey will give the Project Control Board a better understanding of:


  • the ‘change readiness’ of the leaders of this change initiative

  • your assessment of the project’s change readiness, specifically with respect to the change assessment and planning phases of the change process.


For each of the following statements, please select the most appropriate level of change readiness, where 5 is the highest and 1 is the lowest state of readiness.

Note: We are not seeking to identify any individuals from the information provided, we will simply analyse the feedback in relation to the stakeholder group you form part of.
