Dear Participant,
My name is Chris Zavislak and I am a student of American Military University in the Information Technology Department under the supervision of Dr. Denise Eggersman. You are invited to participate in a research project entitled: Information Technology: Security vs. Privacy. The purpose of this study is to determine the American public’s opinion of government surveillance tactics and privacy. The following survey was developed to ask you a few questions regarding security and privacy. Since the creation of the Internet, information technologies have brought up many ethical issues and had transformed society interaction. It is our hope that this information can be used to identify the public’s perceptions of government monitoring because information technology can be used to assist with the infringement of personal rights. There are no identified risks from participating in this research. To learn more about security and privacy, there are many sites and articles on the Internet or you may read more information in regards to the collection of electronic communications from:

This survey is anonymous and participation is completely voluntary. You may refuse to participate without consequences. The survey will take approximately 10 minutes to complete. You will receive no compensation for participating in the research study. Responses to the survey will only be reported in aggregated form to protect identities of the respondents and will be solely used to complete a Master’s level creative project. Neither the researcher nor the University has a conflict of interest with the results. The data collected from this study will be kept in a secure file in electronic format with password protection. The survey results may be shared with my faculty advisor until the completion of the graduation requirements.
