Hello Pack 134 Cub Scout Families
You are invited to participate in our Annual End of Year Pack 134 Family Survey. 
In this survey, all scout families will be asked to complete a survey that asks
questions about what worked and what could be improved for Pack 134.  
It will take approximately 5-10 minutes to complete the questionnaire.

Your participation is completely voluntary. 
However, if you feel uncomfortable answering any questions,
you can withdraw from the survey at any point.
It is very important for us to learn your opinions.

Your survey responses will be strictly confidential
and data will be reported only in the Pack Committee
for planning and improvement for the next school year.
