I am Mak Shuk Mei, a psychology student from Polyu SPEED doing final year project. This questionnaire aims to investigate the relationship between attitude towards movies and attitude towards mental illness and it is used for educational purpose only.
 
 
 
Purpose of the study: The purpose of the study is to investigate the relationship between attitude towards movies and attitudes towards mental illness in Hong Kong young adults.   

Procedures: You will be invited to complete a questionnaire. The questionnaire will take 10 to 15 minutes to complete.   

Confidentiality: All the information collected will be kept completely confidential. It will be used for research purpose only. The data will be anonymized and identified by only in subject code.   

Participation and Withdrawal: Your participation is voluntary. You can withdraw at any time without negative consequences. No risk would be involved.
