Thank you for taking the time to fill out this survey which will help to identify your healthcare needs. I am thrilled to share with everyone that we are looking to form a direct primary care business that will service you or your loved one within the assisted living community. When talking about service, we mean monthly to quarterly visits which provide excellent care. Over the years, we have seen the need for better care across all spectrums. When establishing better care, it should not be by getting you or your loved one to the clinic. It also should not be within a 10 to 15 minute scheduled meeting with a doctor/nurse practitioner. With our approach, we will eliminate the need for transportation or limited visiting times. What makes our care so unique is the doctor coming to the patient and also the lengthy time that will be spent evaluating your needs. We find that if we spend more time, we can better focus on pain and medication management. We also understand that if more time is spent, we can better treat ailments with better results. With that one on one time, it really helps to secure a trusting relationship which is essential for positive health outcomes.
