LS OSA Survey

STUDENT FEEDBACK FORM
AY 2015-2016

This survey aims to gather feedback from students who work closely with the LS- OSA. It consists of three sections, (1) People, (2) Processes and (3) Formation Programs.
This questionnaire takes around 15 minutes to complete. All responses are confidential and will be used for evaluation purposes only.

GUIDELINES IN ANSWERING THIS SURVEY:

Please avoid the following common rating errors:

1. Letting a single strength and/or weakness determine the overall rating of an individual/process/program.
2. Rating near the center of the rating scale for all items.
3. Rating someone more favorably because of friendship or feeling of indebtedness.
4. Basing your ratings only on your most recent experience rather than the whole evaluation period
(Evaluation Period: June 2015 to May 2016).
