You are invited to participate in our survey, which is a part of the "Let's Talk About Chlamydia" health promotion program. This anonymous survey asks questions about your opinion after watching the video "Good pharmacist vs bad pharmacist" that is linked to this survey. It will take approximately 5 minutes to complete. To thank you for your participation, at the end of the questionnaire, you will be asked to provide your email address and a $5 coupon at a local cinema candy bar will be sent to you (You can choose not to receive the coupon code).

Your participation in this study is completely voluntary. There are no foreseeable risks associated with this project. However, if you feel uncomfortable answering any questions, you can withdraw from the survey at any point. It is very important for us to learn your opinions.
