Hello:
The purpose of this survey is to nominate a LEAP Award recipient. It will take approximately 15 minutes to complete the questionnaire.

The LEAP Award is going to be awarded quarterly and nominees must exihibit the following traits in order to be considered.
L - Leadership: Proactively identify & understand the scope of the organizations needs. Empower & encourage our teams to problem solve.
E - Execution: Focus on efficient processes. Execute processes with and understanding of the impact on other areas. Foster a mutual focus on internal controls and deadlines.
A- Accuracy: Report consistent, accurate, and value added information on a timely basis
P- Partnership: Engage stakeholders to understand accounting processes. Effectively collarborate to develop and report financial measures that drive results.

With that in mind please complete the following questionnaire openly and honestly so that Management can select the best candidate for the LEAP Award.
