Thank you for taking part in this trial Artist Group Health Check for Visual Arts Sutherland (VASu). The Health Check was designed by HI-Arts to give your group a chance to reflect on the operation, delivery, access, communication and promotion of your group. The Health Check will enable you to comment and speak freely, and confidentially, on all aspects of your group’s organisation, the results of which will then be fed back to the group through a Health Check Analysis and Report, produced by HI-Arts.
The online questionnaire is made up of a mixture of 39 questions, which comprise of tick boxes, rating scales and open comment sections. You can view the questionnaire as often as you wish, but please only submit one completed questionnaire. Once you have begun answering questions you must complete the entire questionnaire, so that we can effectively capture your feedback. All questionnaires are completed anonymously, to secure the confidentiality of answers and feedback. HI-Arts will not disclose any information gathered through the Health Check process or the Health Check Analysis and Report with any third party or organisation, without the expressed permission of the VASu committee.
