GENERAL INTRODUCTORY QUESTIONNAIRE

Your replies to the questions below assist me to efficiently gather important career-based information about you. This will in turn help me to know how to advise you better.

The questionnaire will take about 15 minutes to complete. Please attempt as many questions as your can before our first appointment. The questions are intentionally general and there are no right or wrong answers.

1. What do you want to achieve from consulting a careers consultant?

3. What are the most important things in life to you?

4. Interests, hobbies, recreational activities - ie what do you like to do in your spare time?

5. What would be your 'dream job', if you were adequately paid and qualifications were not a barrier ?

6. What are your strengths?

7. Are there any 'absolute' requirements you have surrounding your future career? For example, can only work in Auckland to look after elderly parents, salary must be at least $90,000, won't study more than 1 yr, cannot work weekends, must be outdoor work etc.
