My name is Nineveth Fauni, M.A., a Doctoral candidate of the American School of Professional Psychology at Argosy University, Orange County. I am working on a Clinical Research Project under the advisement of Dr. Aldwin Domingo. The study is approved by the Argosy University Institutional Review Board (Protocol number XX.XXX.XX).

You are invited to participate on a study specific to Filipino Americans, with the main goal of understanding the specific cultural factors influencing utilization of mental health care and treatment for this minority group. This survey will take approximately 10 minutes to complete.

Your participation in this study is completely voluntary. There are no foreseeable risks associated with this project. However, if you feel uncomfortable answering any questions, you can withdraw from the survey at any point. While you are free to discontinue your participation at any time, we encourage you to complete the survey questions.

Your survey responses will be strictly confidential and data from this research will be reported only in the aggregate. The survey will not require your signature or identifying name. The survey will not contain any information that could be used to identify you. Your responses will be coded and will remain confidential.
