We are asking for your help in identifying our strengths and to provide feedback so that we may develop our weaknesses. It is important that your answers be honest. The purpose of this process is to help the company improve. Please do not use this form to try to settle a score or badmouth someone. Note that your answers must be based on things that have happened at work only during the period indicated above.

Answers for each supervisor will be grouped. Even though you received an individualized e-mail, your responses will in no way be attributed to you. Comments will be included in summary reports, but will not be linked to an individual respondent.
