Hello and thank you for volunteering this survey. The purpose of these questions is to examine your experience with a leader in the workplace. This could be a boss, supervisor, et cetera- as long as this person held a formally acknowledged position of authority over you.
 This could be someone you currently work under, or someone you worked under in the past. Please answer all questions in the survey according to your experience(s) with ONE specific leader. If you would like to take the survey again in relation to your experience(s) with a different leader, you may do so, but please make sure that each time you take the survey, you continue to answer the whole survey with only ONE leader in mind.
