Dear Manager or Business Owner:
Thank you for your participation in this study - and for your time and support. For your involvement, we will provide you a behavioral blueprint that maps the underlying employee behaviors to the performance levels you provide.
As a manager responsible for overseeing employee's performance and work, you will be completing a general employee appraisal form for each participating employee.
Each employee participant will be provided with behavioral measurement tests in the areas of work styles and leadership preferences. When all participating employees have completed the test(s) provided to them and after you have provided the appraisal information for those participating, we will prepare an analysis and report for you that correlates employee behaviors to their performance levels. For that reason, our only requirement is for you to be brutally honest with your appraisal(s) and answer the questions with preferably no bias.
