You were selected as a participant because of your experience in DE. Please read this form and ask any questions you may have before agreeing to participate in this assessment.

This assessment is being conducted by: Doris Owens.



Background Information:


The purpose of this survey is to determine MSU's e-learning readiness.



Procedures:


If you agree to complete this assessment, you are asked to complete 1 survey based on an extensive literature review.




There is one short-answer question, all other questions are multiple choice and at the end of the survey you may add additional comments. It will take 10-30 minutes to complete depending on your comments.



All questions require a response.


Risks:


Being a participant in this study has no foreseeable risks.



Benefits:


The researcher hopes to identify how MSU defines e-learning and how comfort level with of e-learning.



Confidentiality:


The records of this study will be kept private. I will not include any information that will make it possible to identify a subject. Research records will be kept in a secured file; only the primary researcher will have access to the records.
