You are being invited to participate in a research project conducted by Adam Grayson, who is a doctoral candidate at University of the Rockies.

You are invited to participate in a research study about how supervisor-employee trust and the perceived quality of supervisor-employee coaching relationships influence thriving at work.

You will be asked to participate in an online survey that will take approximately 10 minutes of your time.

The research will explore the quality of supervisor-employee relationships and thriving at work. Aggregate results (containing no personally identifying information) will be shared with the firm to benefit understanding and Level Coach development. Others may benefit by learning from the results of this study.

If you have decided to participate in this project, please understand that your participation is voluntary and that you have the right to withdraw your consent or discontinue participation at any time. You also have the right to refuse to answer any question(s) for any reason.

In addition, your individual privacy will be maintained in all publications or presentations resulting from this study. The data collected in this study will only be used for learning and research purposes. Your individual responses will not be shared with anyone inside or outside of your organization. Each participant, when enrolling to take the online questionnaire, will be given a unique code number to ensure only one set of results is assigned to each participant and all data analysis and results will utilize the unique code numbers rather than personal identifying information. All data collected in this study will be destroyed 5 years after the conclusion of the study.

You will be asked to provide a unique code on the survey that you may provide the researcher if you wish to withdrawal your information, either during the survey or afterwards. If you ask to be withdrawn from the study, your responses will be permanently deleted from the dataset. Any physical copies of the dataset containing your responses will also be destroyed.

The IRB Approval for this study expires on June 14th, 2015.
