Dear Physician,

You are invited to participate in a national survey intended to describe current clinical practice patterns in the radiological workup of children less than 2 years of age with suspected child abuse.

The survey consists of 16 questions about child abuse workup and should take no more than 5-10 minutes for you to complete. Your participation in this study is completely voluntary and you are free to withdraw your participation from this study at any time.

This study has been approved by the Children and Youth Institutional Review Board of the State University of New York at Buffalo. There are no risks associated with participation as the survey collects no identifying information of any respondent and all of the responses to the survey will be recorded anonymously. You will not experience any direct benefits from participation of this research, but your involvement could help us understand better about the practice patterns regarding the workup of suspected child abuse in neurologically stable patients, and the knowledge gained from this study may help us develop practical guidelines for the management of suspected child abuse victims.

If you have any questions regarding the survey or this research project, please contact Dr. Huma Shaikh at [email protected]. If you have any questions concerning your rights as a research participant, please contact the Children and Youth IRB of the State University of New York at Buffalo, 716-878-7141.

By completing and submitting this survey, you are indicating your consent to participate in the study. Your participation is greatly appreciated.

In each of the following scenarios, please check any of the following
studies you would typically order in your real life practice.

* 1. 5 month old female is brought in by her mother with a 3 day history of vomiting and diarrhea, with poor oral intake on day of presentation. Patient has had fevers at home, max 102 F. On exam, patient is afebrile, vitals stable, crying but consolable, anterior fontanelle is flat, and she has mild rhonchi to right lung on auscultation. Chest x-ray is obtained and shows possible old fractures to the 5th and 6th ribs. Which of the following imaging studies would be a part of your initial workup (can choose multiple answers)?

Skeletal survey

CT scan head

MRI scan head

Bone scan

* 2. 18 month old female is brought in by mom with facial bruising. Patient was at home with mom and mom’s boyfriend last night when mom recalls that at 4 am, she heard the baby crying while her boyfriend went to check on her. In the morning, child had swelling to one side of the face and bruising around the mouth. Mom confronted the boyfriend at this time, and then brought in the child to the ER. On exam child appears in no distress, has some swelling to right cheek and mandible with bruising noted around the mouth, no scalp hematoma or palpable fractures. Which of the following imaging studies would be a part of your initial workup (can choose multiple answers)?

Skeletal survey

CT scan head

MRI scan head

* 3. 2 month old male comes in with swelling in his right leg. Mom has noticed that he is not moving it as normal for the last 2 days, and he has been crying with diaper changes. Otherwise he has been acting normal. On exam, vitals are stable; patient is alert, normal exam except for swelling and tenderness in right thigh. Femur x-ray shows distal femur fracture. Which of the following imaging studies would be a part of your initial workup (can choose multiple answers)?

Skeletal survey

CT scan head

MRI scan head

Bone scan

* 4. 2 month old male sent in by PMD; patient has an older sibling who recently presented to the emergency room with seizures and found to have acute head injury and rib fractures suspicious for child abuse. Patient is here for work up. On exam, alert and active 2 month old with no deficits. Which of the following imaging studies would be a part of your initial workup (can choose multiple answers)?

Skeletal survey

CT scan head

MRI scan head

Bone scan

* 5. 21 month old female brought in by mom with swollen arm. Patient stays with a babysitter during the day while mom is at school. Mom was told that the child fell down yesterday, but acted fine afterwards so mom was not concerned. Today when mom was dressing her, she noticed the swelling and bruising to her left arm. On exam the left humerus is tender and swollen, no other injuries. X-ray shows midshaft humerus fracture. Which of the following imaging studies would be a part of your initial workup (can choose multiple answers)?
