Why some changes are successful while some are not?


The aim of this questionnaire is to identify & evaluate the factors contributing to the success or failure of an organizational change initiative. Kindly share your (organization's) experience by marking the necessary box or button, or by entering an answer in the text box wherever applicable.

Kindly note: This questionnaire is part of a dissertation research project conducted by an MBA student from Durham university Business school, UK. All responses will be treated in the strictest confidence - the anonymity of all participants will be maintained.
