Dear Caregiver, 

Many of you have been with us since the start of our Fireworks program in August 2015.
Many of you likewise have ardently supported the critical changes we needed to make. 

Beside being attentive in the service and care for your loved ones, another key focus factors of the Team@TCL is to build close relationships with you, the caregivers. 

We've not conducted any survey of your experience with us and to seek your feedback on areas where we have fallen short of your expectations and could make improvements on as we further our journey with your loved ones. 

We've put together the following survey and would appreciate it if you could take a moment to share with us your opinion and comments.
