As you are all well aware, there have been problems in regards to backing up data/files in your department. This questionnaire will help us determine solutions we can implement in order to prevent any further issues with regards to this matter. This will also help us determine specific needs of the staff in your department as a whole, and individually as a staff member.


Please kindly answer all the questions as accurately as possible so we can develop the most appropriate solutions for this current problem.
