The Attention Center is working on establishing our 501c3 status to become a Nonprofit Organization. We are looking for ways to expand our services along with how we can provide services to those who haven’t been able to afford us in the past. Your responses will be a guide for us to improve our Center. We hope you can help us by answering the following questions as thoroughly and honestly as you can. It should take less than 20 minutes to complete. The time you invest in this survey will bring us closer to achieving our goal, which is to provide more services to more people in need. Thank you for taking the time to take this survey. If you have any questions please contact us at 216-901-2300 or email at [email protected].
