Hello:
You are invited to participate in our survey to assess the needs of teachers working with students with emotional and behavioral disabilities. In our project, approximately 75 people will be asked to complete a survey that asks questions about their experience working in public elementary schools. It will take approximately 10 minutes to complete the questionnaire.

Your participation in this study is completely voluntary. There are no foreseeable risks associated with this project. However, if you feel uncomfortable answering any questions, you can withdraw from the survey at any point. It is very important for us to learn your opinions.

Your survey responses will be strictly confidential and data from this research will be reported only in the aggregate. Your information will be coded and will remain confidential. Students that suffer from Emotional/Behavioral Disorders are said to exhibit, but are not limited to, certain types of behaviors: disruptive, impulsive, preoccupied, aggressive towards peers, self harming behavior, low self esteem, and resistance to change. For the purpose of the next series of questions, imagine having a student with an EBD diagnosis in your classroom.

Using the 'Star Rating' scale, where 1 is the least and 5 is the most, please rate the following based on how strongly you agree with each statement:
It would be academically advantageous for this student to be in the general education setting.
It would be academically advantageous for the other, typically developing students to have this child in the general education setting.
This student would be socially accepted by the other students in the class.
I would feel confident teaching this student in my classroom.
If this student becomes disruptive and noisy, I feel assured that I know the techniques to redirect him quickly.
I would be able to adapt my lessons to meet the needs of this student, while also appeasing the typically developing students.
