The Simcoe County District School Board is dedicated to providing the highest quality psycho-educational assessments possible. Therefore, we are collecting information to ensure the effectiveness of assessment services as well as the resulting programming outcomes.
Our records indicate that a psycho-educational assessment was recently provided to a student from your school and that a feedback session was scheduled for today. We ask for your support in completing this short questionnaire regarding the assessment and feedback session.
Please be advised that your responses are anonymous and confidential. All responses from schools will be combined into a summary report solely for the purpose of ensuring the effectiveness of assessment services. No information regarding either your school or the student will be collected or used for any other purpose.
