“An Investigation into different health professionals and patients perceptions of different types of medical error”

We would like to invite you to take part in our research study. Before you decide, it is important for you to understand the purpose of the research and what it will involve. Please take time to read the online information carefully and discuss it with friends and relatives, if you wish. Please email the address at the bottom if there is anything that is not clear, or if you would like more information.

What is the purpose of the study?
The purpose of this study is to determine how healthcare professionals and patients perceive medical errors in different situations.

Why have I been invited?
All healthcare professionals are invited to complete the questions, as well as student doctors/nurses and members of the public who have been patients. This is to be able to gain an insight into medical errors which occur within healthcare.

How long is the study?
The study should take no longer than 5 minutes.

Do I have to take part?
No, it is optional for people to take part and you can withdraw from the study at anytime by exiting the programme.

What will I be asked to do if I take part?
If you chose to take part, you will first be presented with 4 different scenarios to comment on. You will then be asked four short quick questions on medical error and finally be first asked a few personal questions such as you gender, age and job role. All information is confidential.

What are the possible disadvantages and risks of taking part?
Some people may experience mild distress due to the sensitive nature of the topic; however in the rare occasions that this does occur we recommend you exit the programme. We have provided details of support agencies that can help provide support.

How will I benefit from participating in this study?
You won’t benefit directly however you will be helping identify potential issues within the healthcare system which could led to improvements.

Confidentiality
All information provided is confidential, and all information is stored on a secure password protected data base.

What will happen if I don’t want to carry on with the study?
You can withdraw at any point during the study; all information provided will not be used in the research.

How will we use the results of this research?
The results will be used to determine how healthcare professionals and patients perceive medical errors in different situations. These results will be written up in a research project.

Can I request the results of the study?
Yes, you can request the results of the study by emailing the address below from September 2011 onwards.

This study has been reviewed and given a favourable opinion by the Institute of Work Health & Organisations Research Ethics Committee.

A normally healthy 23 year old man goes to hospital for surgery, the doctor doesn’t noticed the patients allergy information in his medical notes and prescribes antibiotics to be given after surgery. The patient wakes up gasping for air. It is quickly realised that the patient is allergic to the antibiotics and they are stopped. Despite this the patient stops breathing. The doctor and his team where unable to resuscitate him and the patient dies.
