Hello: You are invited to participate in my survey, Investigation of supply chain risk and sustainability faced by Pentagon Freight Services (PNG) while operating in Papua New Guinea.
In this survey, approximately 25 people will be asked to complete a survey that asks questions about the effects of corruption on the sustainability of the supply chain in PNG. It will take approximately 10 minutes to complete the questionnaire. Your participation in this study is completely voluntary. There are no foreseeable risks associated with this project. However, if you feel uncomfortable answering any questions, you can withdraw from the survey at any point. It is very important for me to learn your opinions. Your survey responses will be strictly confidential, and data from this research will be reported only in the aggregate. Your information will be coded and will remain confidential. * • What do you think an average wage should be to afford you and your family a comfortable lifestyle?

• As PFSPNG have 60 staff, how do you think the organisation can afford to pay those wage levels?

* • Looking at corrupt activities as a third party, what do you think drives these corrupt or opportunistic behaviours?

* • As most theft, in any form, is usually discovered and results in dismissal, why, in your opinion do you think the individual is thinking when he commits these acts knowing that future income will be lost as a result?

* • What is the impact on the supply chain in PNG by paying bribes and incentives to Government officials?
